- The Medial Head: Originates on the back of the upper arm (humerus).

Collectively, all three heads travel down the arm, cross the elbow joint, and insert into the forearm. Because of their attachments, all three heads perform elbow extension (straightening the arm from a bent position).
However, because the long head also crosses the shoulder joint, it performs a second primary function: shoulder extension (bringing the arm from overhead down to the side and behind the body).
How Shoulder Angle Influences Triceps Recruitment
While all three heads perform elbow extension, different heads can be prioritized during movement. This occurs due to differing length-tension relationships, angles of attachment, and possible differences in nerve innervation.
Research shows that altering the shoulder angle directly changes the force output of each head:
- Long Head: Produces the most force when the arm is positioned down by your side. Force production drops off significantly as the arm is raised overhead.
- Medial Head: Operates in reverse; it produces the most force when the arm is elevated overhead and the least when the arm is by the side.
- Lateral Head: Shows a force profile similar to the medial head, though its changes across shoulder angles are less extreme.
What About the Classic Bodybuilding Theory?
Classic bodybuilding logic often claimed that overhead extensions stretch and emphasize the long head. Research has not supported this theory. Because most bodybuilders historically performed both overhead and down-by-the-side movements, this assumption was rarely stringently tested. The data indicates the opposite: the long head produces force best with the arm down by the side.
Best Isolation Exercises for Each Triceps Head
Targeting the Long Head: Cable Pressdown
Because the long head prefers the arm positioned by the side, a pressdown is ideal.
- Setup: Lean slightly forward, hinge at the hips, and keep your elbows tucked in tight to your sides.
- Execution: Extend through the elbows.
- Cue: Keep your palms facing in rather than rotating out at the finish to maintain a direct line of action.

Targeting the Medial and Lateral Heads: Overhead Extension
To emphasize the medial and lateral heads, use exercises where the shoulder is flexed to a higher range.
- Modality: Can be performed with dumbbells, cables, or resistance bands.
- Execution: Single-arm variations are preferred. Training one arm at a time allows for better positional adjustments, controlled movement, and the ability to sink deeper into shoulder flexion.

Advanced Long Head Variations
Because the long head performs both elbow extension and shoulder extension, adding movements that combine both actions increases the training stimulus on this head.
- Rock-Back Pressdown: Set up in a split stance. As you press down into elbow extension, rock back slightly to become more upright at the end of the range. Performing this variation with one arm at a time improves coordination and trunk stability.
- Single-Arm Cable Kickback/Pressdown Hybrid: Set up in a split stance, lower the cable attachment point, hold the upright frame with your off-hand for stability, and perform a single-arm extension. This setup provides consistent cable tension at both the bottom and top of the range, solving the resistance drop-off seen in traditional dumbbell kickbacks.

The All-in-One Option: Supine Extension with Active Pullover Hold
If you want a single isolation movement to train all three heads comprehensively:
- Exercise: Supine Triceps Extension with an Active Pullover Hold (a skull crusher variation with elbows angled back).
- Mechanics: Positioning the arms in higher shoulder flexion places high demand on the medial and lateral heads, while actively holding the position against shoulder extension forces the long head to work harder than in a standard skull crusher.

Compound vs. Isolation Movements: What the Research Shows
A study by Brandão et al. (2020) evaluated whether compound pressing or isolation exercises build more tricep mass across three distinct groups:
- Lying Tricep Extensions Only: Effectively grew the long and medial heads, but produced poor growth in the lateral head.
- Bench Press Only: Significantly grew the lateral head, produced some medial head growth, and resulted in minimal growth for the long head.
- Combined Group (Bench Press + Extensions): Produced the best overall hypertrophic results across all three heads, particularly when the bench press was performed before the isolation exercises.
